Assessment and Ongoing Support
Our Registered Nurses can undertake a health assessment to help determine your care needs – please contact us. This can be done as part of a Government funded package or privately. Once completed, the health assessment can be used to help you to decide what services you may need to maintain your independence and ensure that you can continue to live at home with the help you need.
Care Planning, Case Management and Community Resource Coordination
Once an assessment has been completed, a Registered Nurse will develop a care plan for you in conjunction with you and your family. This will detail the services that you have identified and the days and times that you prefer. We will then arrange these services, which may include personal care, cleaning or gardening services, meal provision, or medication administration. The Registered Nurse is your key liaison point ensuring continuity of care and familiarity for you and your family, and ensures personalized management of your care plan. Your plan will be reviewed every 6 months or as your needs change.
General Nursing Services
Our nursing staff provide a full range of nursing services in the home, ranging from showering assistance to blood transfusions. They will liaise with the doctor and assist with the coordination of medical and other allied health professional appointments. We can advise on products and support for you in the following areas:
- Continence management & Assessments
- Catheter care, Changes and management Diabetes management – including insulin administration. Wound management Medical management and administration including sourcing and picking up medication, administering drugs, IV etc
- Our nurses are also able to provide advice and education on appropriate safety equipment for the home, and refer to an Occupational Therapist for specialist equipment if it is identified that this is required. Palliative Care Palliative Care is the specialised, multi-disciplinary, and holistic care of someone suffering from an advanced, progressive terminal illness. Our nursing staff and support workers will provide health care support services, medication administration, and comfort and advice to palliative care patients and their families. We work closely with your Palliative Care Team and Community Nursing Team in a multi-disciplinary approach to your care. Our palliative care services are usually upon referral, but may also be privately commissioned. Education for Client/Carer Provision of information and support to the patient and their carers, including lifestyle maintenance, nutritional advice, ‘Preparation for the Day’ planning etc.
